As a 'convenience', they have included a form to 'allow' me to supply them with some basic information. Notice that if I check off any of the blocks I will be stating that I am a taxpayer and, thus, liable for filing the tax return and for paying tax according to the IRS rules. Notice also that there is an affidavit for me to sign at the bottom of the form. As I said, it is critical that they get me to sign this affidavit.
